TRAINING CLASSES -- REGISTRATION FORM

The Registration form allows up to three persons to register for the same classes (up to four) -- as long as all of them plan to attend all the classes selected

Otherwise fill out the form once for each person & class by clicking on Return to Form at the bottom of the Confirmation Page

When the confirmation page appears, you may want to print it for your records -- Simply click on the printer symbol on the toolbar (at the top of the screen) or choose File, Print from the menubar.

The email address you provide will be used to send you additional class materials and/or notice, if the class has to be cancelled. Include your phone number if that is the easiest, quickest way to reach you in case of cancellation.
